General comments
Aangepaste dosis bij overgewicht
Cefazoline bij patiënten met een BMI > 40: 3 gram cefazoline preoperatief; bij herhaling kan 2 gram worden toegepast.
Vancomycine bij patiënten met een totaal lichaamsgewicht boven de 100 kg: 10 mg/kg totaal lichaamsgewicht.
Herhaalde profylaxe
Een hernieuwde dosering perioperatieve antibiotische profylaxe is nodig bij een operatieduur van > 2 keer de halfwaardetijd van het antibioticum (voor cefazoline bij operatieduur > 3 uur, clindamycine operatieduur > 5 uur, cefuroxim operatieduur > 3 uur) of bij bloedverlies van > 1500 ml.
Herhaalde profylaxe met vancomycine is in de regel niet nodig.
