General comments
Een laparoscopie met laag infectie risico wordt gedefinieerd als: een korte (< 1 uur) procedure, die ongecompliceerd verloopt, bij een patiënt onder de 70 jaar, waarbij geen spil optreedt van cyste-inhoud, waarbij bloedverlies beperkt (< 500 ml) blijft, waarbij geen implantaat, mesh of ander onoplosbaar materiaal gebruikt wordt.
In alle andere gevallen wordt de ingreep beschouwd als een laparoscopie met hoger infectie risico.
Aangepaste dosis bij overgewicht
Cefazoline bij patiënten met een BMI > 40: 3 gram cefazoline preoperatief; bij herhaling kan 2 gram worden toegepast.
Clindamycine: bij patiënten met een lichaamsgewicht > 180 kg de dosering clindamycine verhogen van 600 mg naar 900 mg.
Herhaalde profylaxe
Een hernieuwde dosering perioperatieve antibiotische profylaxe is nodig bij een operatieduur van > 2 keer de halfwaardetijd van het antibioticum (voor cefazoline bij operatieduur > 3-4 uur, clindamycine operatieduur > 5 uur) of bij bloedverlies van > 1500 ml.
Herhaalde profylaxe met metronidazol is alleen nodig bij >1500 ml bloedverlies.
