Priority: 1st choice	Medication: cefazoline iv 1000mg 3dd	Remarks: Bij <1 jaar geleden een "delayed type" allergische reactie (maculopapulaire rash) op penicilline/ amoxicilline
Priority: 2nd choice	Medication: cefuroxim iv 1500mg 3dd
Priority: 2nd choice	Medication: ceftriaxon iv 2000mg 2dd	Remarks: ceftriaxon 2 dd
Priority: 2nd choice	Medication: piperacilline/tazobactam iv 4000/500mg 4dd or piperacilline/tazobactam iv loading dose 4500mg, 18000mg continuous per 24 hours
Priority: 2nd choice	Medication: meropenem iv 1000mg 3dd or meropenem iv loading dose 1000mg, 3000mg continuous per 24 hours

≥ 18 years

Indications: sterk vermoeden op- of bewezen cefalosporine-en meropenem-allergie of een eerdere ernstige 'delayed type' allergie op een ß-lactam antibioticum

Het toevoegen van aminoglycosiden wordt NIET aanbevolen

Onderhoudsdosering vancomycine afhankelijk van gemeten spiegels en van nierfunctie, zie bij vancomycine en bij TDM vancomycine

Behandelduur 14 dagen bij ongecompliceerde S. aureus bacteriëmie, bij gecompliceerde S. auereus bacteriëmie behandelduur 4-6 weken
